Travel Risk Assessment Form

Get advice about vaccinations at least eight weeks before you’re due to travel. If you do need new vaccinations, some jabs need to be given well in advance so that they can work properly. When you have made an appointment with a nurse for any immunisations or advice please click on the Travel Risk Assessment Form header (above), print off and complete the form and then bring this along to your appointment.
